Infertility is a serious condition that can affect both men and women. Causes include low sperm count, poor sperm quality, and pre-existing medical conditions. Various assessments help identify the root cause of failed pregnancies.

Personal Medical History

Doctors will inquire about family history, sexual practices (timing, lubricant use), and medical history, such as precocious puberty, STDs, or urinary tract infections, as these can significantly impact fertility.

Physical Examination

One of the first steps in diagnostics is a physical examination, including a scrotal test and general assessment of body habitus to check for proper androgen-dependent masculinization.

Diagnostic Procedures

  • Semen Analysis: The most important test. Examines sperm count, shape (morphology), movement (motility), and presence of infections. Several tests may be performed for accurate results.
  • Hormone Analysis: Evaluates levels of hormones produced by the pituitary gland, hypothalamus, and testicles, which are vital for sexual development and sperm production.
  • Testicular Biopsy: Used when sperm production is suspected to be normal but a blockage in sperm transport is likely.
  • Specialized Tests for Sperm Function: Rare tests to determine how well sperm survive and penetrate an egg.
  • Genetic Analysis: Examines DNA to determine if gene changes are causing infertility issues.

Essential Terms to Know

AMH (Anti-Mullerian Hormone)

Female Fertility Marker

Produced by cells in ovarian follicles. A high level indicates a large number of follicles/eggs (good ovarian reserve). Levels naturally decrease with age.

FSH (Follicle Stimulating Hormone)

Female & Male Fertility Marker

In women, it's responsible for egg maturation. High FSH levels (tested day 2/3 of cycle) indicate poor ovarian reserves. In men, it promotes healthy sperm production.

AFC (Antral Follicle Count)

Female Fertility Marker

The most accurate indicator of ovarian reserves. Measures the number of antral follicles (containing immature eggs) via vaginal ultrasound. High count suggests significant ovarian reserve.

LH (Luteinizing Hormone)

Male Hormone Marker

Stimulates testosterone production in men. An increase in LH level can indicate that the testicles are not producing enough testosterone.

Testosterone

Male Sex Hormone

Controls masculine characteristics. Unusual decline in levels can lead to infertility and is evaluated as part of a complete hormone profile.

Prolactin

Male Hormone Marker

Produced by the pituitary glands. Increased levels can be linked to erectile dysfunction, fertility issues, and lack of energy in men.
